CPSC, Verizon Wireless Announce Recall of Counterfeit Cell Phone Batteries
The recalled batteries can be found in LG TM-510 model cell phones, have a manufacturer/date code of "AEMLLL 02220," AEMMHH 02220," "AEMLLL 02X25H" or "AEMMHH 02725" and were distributed by Verizon Wireless. The manufacturer/date code can be found on the battery, below the words, "Model/Modelo" or "LG." The phone's model number appears on the back side of the phone beneath the battery. The cell phones, which come in black or silver, may have the Verizon Wireless name on the front.
CPSC, L. Powell Co. Announces Recall of Glider Rockers
These are "Nostalgic Oak" Bentwood Glider Rockers with models numbers 139K, 139L, 144K and 144L. They move back and forth, using metal bearings. The model number is written only on the packaging. A label on the underside of these chairs reads, "Powell" and "Taiwan," and a white factory code sticker on the underside of these chairs reads "TZ." Rockers sold before November 1999 are not included in this recall.
CPSC, Lex Pharmaceutical Inc. Announce Recall of Vitamins and Medicines
July 21, 1997 Release # 97-161 WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Lex Pharmaceutical Inc. of Medley, Fla., is recalling about 25,000 bottles of vitamins and medicines because they do not have child-resistant packaging as required by the Poison Prevention Packaging Act. If accidentally swallowed by children, these products can cause serious injury or death.CPSC is not aware of any injuries involving these products. However, about 50 children each year die from poisoning after swallowing medicines and other toxic substances.This recall is being conducted to prevent the possibility of injury. The recalled products all have plain, screw-on caps. The recall does not involve units with child resistant closures. The recalled products were packaged by Lex Pharmaceutical Inc., but some have different distributors. The recalled products are: - Multigon Junior, a 240-count bottle of chewable vitamin mineral supplements for children, comes in a clear bottle with a white label showing a spotted orange cat on the front. It is distributed by Menper Distributors of Miami, Fla. - Duralgina, a 30-count bottle of analgesic tablets containing acetominophen, also is distributed by Menper Distributors of Miami, Fla. It comes in a white bottle with orange lettering with "THE SUPER STRENGTH PAIN RELIEVER" written on the front. - Cramps Out, a 24-count bottle of tablets containing acetaminophen. It comes in a white and orange bottle packaged in a box. The words "SLEEP AID AND PAIN RELIEVER" and a diagram of a knotted rope within a leg are on the box and bottle. It is distributed by Efficient Laboratories of San Juan, Puerto Rico. - Uni 1 Dose, a 100- and 365-count bottle of multiple vitamins with iron that has a blue label on a white bottle (100-count) or clear bottle (365-count). It is distributed by Medley Pharmaceutical of Medley, Fla. - Lexagran-M Vitamins, a l00- and 130-count bottle of tablets, comes in a white bottle with an orange and blue label design. The word "LEX" is written above the product name. It is distributed by Lex Pharmaceutical. - Hematinic Beef-Iron and Wine, a 16-oz. white or brown bottle contains this liquid supplement which also has "LEX" written above the product name with an orange and blue label design, and is distributed by Medley Pharmaceutical. - Supra-Vita, a 100-count bottle of vitamin tablets, comes in a white bottle with a pink label. "Supra-Vita" is written in green and most other writing on the bottle is purple. It is distributed by Carifarma Pharmaceutical Inc. of Rio Piedras, Puerto Rico. The vitamins and medicines were mainly sold at small pharmacies, and discount drug and retail stores in Puerto Rico, New York, Florida and the Southeastern United States from January 1996 through June 1997 for about $5.Consumers should immediately discontinue use of the recalled products and return them to the store where purchased for a full refund. Mini-Hammocks Recalled by 10 Manufacturers NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE CONTACT: May 16, 1996 (301) 504-7908 Release # 96-125 CPSC and 10 Manufacturers Announce Recall of 3 Million Mini-Hammocks WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), 10 mini-hammock manufacturers and importers are recalling over three million light-weight, net mini-hammocks that do not have spreader bars. Without spreader bars to hold the mini-hammock bed open, the mini-hammocks can twist around children's necks as they are getting into or out of the mini-hammock, resulting in strangulation and death. When a net mini-hammock is attached to trees, decks, porches, or other areas, it hangs like a thin rope. The mini-hammock can suddenly become twisted around a child's neck and strangle him. This can happen when children are attempting to climb into or out of, are playing on, or are swinging on mini-hammocks like swings. Between 1984 and 1995, CPSC received reports of 12 children between the ages of 5 and 17 years old who became entangled and died when using net mini-hammocks without spreader bars. CPSC is also aware of an injury to a seven-year-old girl who suffered permanent brain damage from a near-strangulation in a mini-hammock. Another near-fatal incident involved a five-year-old boy who was found entangled in a mini-hammock but was resuscitated by his mother. Mini-hammocks are light-weight, portable, thin net hammocks that are made without spreader bars. They measure between five to seven feet wide and seven to twenty feet long. Sporting goods stores, outdoor equipment stores, Army and Air Force Exchange Service stores ("PX/BX"), department stores, and even drug stores have sold the hammocks since the early 1970s for about $4 to $10. The following manufacturers and importers are participating in this voluntary recall of net mini-hammocks: Academy Broadway, Smithtown, N.Y. Nelson/Weather-Rite, Lenexa, Kan. Algoma Net Company, Algoma, Wisc. Rothco, Smithtown, N.Y. Avid Outdoor, Olathe, Kan. Schwarzman Export Import Co., Brooklyn, N.Y. Coghlan's, Minneapolis, Minn. Standard Sales, Los Angeles, Calif. E-Z Sales, Gardena, Calif. Texsport, Houston, Tex. E-Z Sales, the major producer of these thin, net mini-hammocks, is offering consumers replacement mini-hammocks with spreader bars or larger hammocks for returning their MiniPak Hammocks and Superhammocks. The nine other manufacturers participating in this recall are offering a full refund for returning their mini-hammocks to the store where purchased. Recall notices offering consumers refunds or replacement hammocks are posted in stores that sold the recalled mini-hammocks. Consumers should immediately remove mini-hammocks from porches, decks, trees, recreation rooms and other locations. Consumers should return the mini-hammocks to the store where they were purchased for a full refund or a replacement hammock. If consumers do not know the retailer or manufacturer of their net mini-hammock, consumers should destroy the mini-hammock immediately to prevent a strangulation death or injury.

May 4, 1994 Release # 94-070 PRODUCT: Approximately 14,400 electric potpourri pots sold at Walgreen stores prior to June 1, 1992. The white ceramic pots have a blue colored design on the side of the pots. This design can be a wreath, a flower basket, sea shells, or fruit. The pots have a blue ceramic lid and a white electric power cord. The pot holds approximately one and a half cups of water. The recalled pot has a black plastic bottom and is labeled on the bottom "model WA-00117" and does not have a UL listing mark, although there may be a UL mark on the pot's power cord. PROBLEM: The electric potpourri pot presents an electric shock hazard if a metal heating element retaining bar inside the sealed base of the pot comes in contact with the unit's power cord terminals. This could permit metal nuts on the outside bottom of the pot to become electrically charged. No incidents of electric shock have been reported to either the Walgreen Company or CPSC. Other retailers may have imported the same potpourri pots. Consumers who suspect that they may have purchased the same model electric potpourri pots from different retailers are requested to call the CPSC hotline at (800) 638-2772 to assist the agency in identifying other possible retailers of this hazardous product. If the pot is marked as model WA-00117 and was purchased at a Walgreen store prior to June 1, 1992 the pot should be returned to Walgreen for a refund or exchange (consumer's option).
